Beaumont Rehabilitation and Skilled Nursing Centers take an interdisciplinary approach to comprehensive restorative and medical care. Rehabilitation, post-surgical, short- and long-term care, skilled nursing and complex medical management are all provided in our warm, patient-centered environment. Locations in Natick, Northborough, and Westborough.

Description: Administers and coordinates nursing care for assigned patients. Has responsibility for assigned patients, ensuring quality care, and supervision of personnel.

Responsibilities:

Conducts job responsibilities in accordance with the standards set forth in the SALMON Health and Retirement employee handbook, Beaumont Rehabilitation and Skilled Nursing Center policies and procedures, applicable federal and state laws and applicable professional standards.
Keeps confidential all patient information received.
Responsible for documentation of patient condition; records in the clinical record and reports all observed symptoms, reactions, treatments and changes in the patient's condition.
Administers and charts medications as ordered by physician.
Assists physician with examination, treatments, special tests and procedures as defined within scope of practice.
Obtains, carries out and follows physician orders.
Directs patient care for assigned patients.
Orients new admissions and personnel to neighborhood.
Manages nursing neighborhood which includes planning daily and/or weekly work assignments and breaks. Ensures adequate staffing and directs duties of CNAs and nursing assistants
Communicates effectively with patients and their families, physicians and staff.
Participates in team conferences, development of nursing care plans, and team and shift reports.
Answers call lights, assists patients with personal hygiene and other activities of daily living.
Assists in assessing, admitting, discharging and transferring patients.
Complies and performs within scope of practice as defined by state Nursing Practice Act.
Completes all documentation as required by local, state and federal guidelines.
Performs other duties as assigned.

Requirements:

Graduate of LVN/LPN accredited School of Nursing.
Complies with all state licensing standards.
Ability to communicate in English effectively through verbal and written means.
Ability to work under stress and in emergency situations
