Job Summary: Provides strategic direction consistent with Corporate and Department/Program mission, vision, and values for Case Management services. Maintains 24-hour accountability for inpatient/observation discharge planning, coordination, and transitions and all Social Work functions for CRMC Acute Care/ED sites, including all ED psych transfers to other inpatient settings. Ensures continuity of services and safe, seamless transitions in all post-Acute settings including Home Health Care, LTACH, Acute Rehabilitation, Skilled Nursing Facilities, and Primary Care. Responsible for Indigent Services including budget, grant funding, and transition contracts. Responsible for Medical and dental appointment contract with Kintegra. Facilitates management of complex, high length of stay patients and leads multidisciplinary teams to identify solutions. Maintains outcome-based metrics and identifies trends (LOS, Readmissions, Medicare Ratings). Has oversight for high-risk admissions, readmissions, and works with Primary Care Service Line and Care Navigation department to optimize post-acute services for complex patients. Serves as liaison to SNFs, ALFs, HHAs, Kintegra, DSS, and other Community agencies. Identifies and implements contracts with SNF s to be ACO SNF affiliates for patients attributed to CaroMont s ACO. Serves on boards as appropriate. Responsible for grants for chronic disease management, cremation, and indigent services. Collaborates with pastoral care and DSS for unclaimed bodies in need of final disposition. The Director will be responsible for coordinating and managing the departments they support for IT/EMR access, documentation, planning, projects, and implementation of processes. Director will act as the expert for Case Management across all areas within CRMC acute sites. The Director must demonstrate expert skill in building and maintaining collaborative, constructive relationships with CaroMont Health, multiple community agencies and partners.
Qualifications: Master s degree in management, business administration, nursing or related field from an accredited college or university required. BSN and current unencumbered RN license to practice in NC required. Requires a minimum of 5 years of leadership experience in healthcare, 5 years experience in Case Management,: Case Management Certification preferred. Thorough knowledge of Case Management, Social Work, Discharge Planning, Quality, Community Agencies, and regulatory agencies such as CMS conditions of participation and TJC. Demonstrated competence in change management, while utilizing creativity, skill, and knowledge to inspire excellence. Effective negotiating, coaching, mentoring and conceptual thinking skills. Ability to clearly direct and motivate individuals while fostering a collaborative, team-oriented environment. Ability to work effectively with groups and organizations as a leader or a team player.
