Job Summary
The incumbent in this position is a professional clinical social worker who provides direct services to the patients of the Small Bowel Transplant unit. These services are related to hospitalization, dealing with illness and post-hospital planning. The incumbent also provides social work consultation, leadership and teaching functions for the patients of the Small Bowel Transplant unit and their families/significant others as well as for the community. Adheres to NASW Code of Ethics and exemplifies the qualities and values of the social work profession. These functions are performed in accordance with all applicable laws and regulations and MedStar Georgetown University Hospital's philosophy, policies, procedures and standards.
Minimum Qualifications
CONSIDERATION WILL BE GIVEN TO AN APPROPRIATE COMBINATION OF EDUCATION/TRAINING AND EXPERIENCE.
Education/Training
Master's of Social Work from an accredited training program.
Experience
A minimum of 2 years experience with acute and chronically ill patients. Hospital setting preferable. Experience with oncology patients helpful but not required.
License/Certification/Registration
D.C. license at the LICSW level.

Primary Duties and Responsibilities

Provides clinical social work services to identified patients and families of the Small Bowel Transplant unit. This includes outpatient and inpatient services. Includes individual and group counseling regarding the impact of illness and hospitalization.

Collaborates with other health care professionals as part of a multi-disciplinary team. Facilitates communication regarding relevant psychosocial issues of patients and families to health care team through recording in medical records (inpatient and outpatient), consultation and participation in relevant meetings.

Functions as a coordinating link between patient, family, hospital and community resources to assure continuity of care.

Represents social work perspective and serves as department representative to designated Hospital and Small Bowel Transplant committees and community organizations.

Participates in orientation, training, and education programs for students as well as physicians, nurses and other health care professionals as required.

Assumes responsibility for identifying training needs and locating resources and promoting own professional development.

Collaborates with other disciplines in development of new programs that enhance the quality of services to the Small Bowel Transplant unit patient and their significant others.

Collaborates with appropriate clinical staff and assists Discharge Planning Case Management Staff to facilitate appropriate placement of patients. This may be post hospitalization or when clinical conditions warrant.

Actively cooperates as a participant in the Social Work Department, assuming backup-up and on-call responsibilities, committee work, research, QA/QI, and other duties as appropriate.
